Summary
Phenytoin is a strong inducer of CYP3A4 and can substantially lower oxycodone concentrations, causing loss of analgesia or withdrawal symptoms. When phenytoin is stopped, oxycodone levels can rise and cause toxicity.
What this interaction means in plain words
Oxycodone and Phenytoin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Oxycodone belongs to Musculoskeletal; Phenytoin belongs to Central Nervous System.
Why it happens
Phenytoin induces CYP3A4, increasing oxycodone clearance.
What to do
Monitor analgesic response and withdrawal signs and increase oxycodone dose as needed, or consider another opioid. Reduce the dose and monitor for respiratory depression if phenytoin is withdrawn.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
