Summary

Corticosteroids combined with non-depolarizing neuromuscular blockers, especially in prolonged ICU use, have been linked to acute quadriplegic myopathy. Corticosteroids may also alter the duration of neuromuscular blockade.

What this interaction means in plain words

Pancuronium and Prednisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Pancuronium belongs to Musculoskeletal; Prednisone belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive muscle injury and neuromuscular dysfunction; the precise mechanism is not fully established.

What to do

Use the shortest duration of neuromuscular blockade possible and monitor depth of block with train-of-four. Assess for persistent weakness and check creatine kinase if myopathy is suspected.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
