Summary
Pexidartinib is a CYP3A4 inducer and may speed up the metabolism of the R-enantiomer of warfarin. This could reduce anticoagulant effect and lower the INR. The overall effect is expected to be modest since the more potent S-enantiomer is cleared mainly by CYP2C9.
What this interaction means in plain words
Pexidartinib and Warfarin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Pexidartinib belongs to Oncology; Warfarin belongs to Cardiovascular.
Why it happens
Induction of CYP3A4 (and possibly other enzymes) increases warfarin clearance.
What to do
Monitor INR more often when pexidartinib is started, changed or stopped. Increase the warfarin dose if the INR falls below target.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
