Phenytoin induces CYP3A4 and CYP2B6, which can lower tramadol and active metabolite exposure and lessen pain relief. Tramadol can also provoke seizures, which may be harder to judge in patients taking an anticonvulsant.

What this interaction means in plain words

Phenytoin and Tramadol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Phenytoin belongs to Central Nervous System; Tramadol belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Phenytoin enzyme induction increases tramadol metabolism.

What to do

Monitor analgesic response and titrate tramadol upward if needed. Watch seizure control, and reassess the dose if phenytoin is stopped.

What to ask your pharmacist
