Summary

Using a systemic corticosteroid together with an NSAID raises the likelihood of gastric irritation, peptic ulceration and gastrointestinal bleeding beyond that of either drug alone. Piroxicam carries a relatively high GI risk among NSAIDs, and the risk is greater in older patients and with prolonged use.

What this interaction means in plain words

Piroxicam and Triamcinolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Piroxicam belongs to Musculoskeletal; Triamcinolone belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive gastrointestinal mucosal injury: NSAIDs inhibit protective prostaglandin synthesis while corticosteroids impair mucosal healing and repair.

What to do

Use the lowest effective doses for the shortest time and consider gastroprotection with a proton pump inhibitor in patients at risk. Advise patients to report black stools, hematemesis or persistent abdominal pain.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
