Summary

NSAIDs can blunt the antihypertensive effect of angiotensin receptor blockers. In older or volume-depleted patients the combination can cause acute kidney injury and hyperkalemia.

What this interaction means in plain words

Piroxicam and Valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Piroxicam belongs to Musculoskeletal; Valsartan belongs to Cardiovascular.

Evidence level: High

Why it happens

NSAIDs reduce vasodilatory renal prostaglandins, and with angiotensin II blockade this lowers glomerular filtration and promotes fluid and potassium retention.

What to do

Maintain hydration and monitor blood pressure, serum creatinine and potassium after starting the combination and periodically afterward. Avoid in dehydrated patients and consider a non-NSAID analgesic.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
