Summary

Pramipexole is cleared mainly by renal tubular secretion through the cationic transport pathway. Quinidine may compete for this pathway and raise pramipexole exposure. This can increase dopaminergic effects such as dizziness, somnolence, hallucinations and dyskinesia.

What this interaction means in plain words

Pramipexole and Quinidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Pramipexole belongs to Cardiovascular; Quinidine belongs to Anti Parasitic.

Evidence level: Moderate

Why it happens

Competition for renal cationic transport reduces the renal clearance of pramipexole.

What to do

Monitor for dopaminergic adverse effects such as sedation, dizziness and hallucinations. Consider reducing the pramipexole dose if they occur.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
