Summary
Primidone is metabolized to phenobarbital, a potent enzyme inducer that can substantially lower tacrolimus concentrations. This risks subtherapeutic immunosuppression and graft rejection in transplant recipients.
What this interaction means in plain words
Primidone and Tacrolimus can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Primidone belongs to Central Nervous System; Tacrolimus belongs to Miscellaneous.
Why it happens
Primidone and its metabolite phenobarbital induce CYP3A4 and P-glycoprotein, increasing tacrolimus clearance.
What to do
Prefer a non-inducing anticonvulsant where possible. If the combination is needed, monitor tacrolimus troughs closely and increase the dose as required, with readjustment if primidone is stopped.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
