Summary
Pyrazinamide and its metabolite pyrazinoic acid reduce renal urate excretion, which opposes the uricosuric action of probenecid. Hyperuricemia and gout flares may occur, and the urate-lowering effect of probenecid can be blunted.
What this interaction means in plain words
Probenecid and Pyrazinamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Probenecid belongs to Genitourinary; Pyrazinamide belongs to Anti Bacterial.
Why it happens
Pyrazinoic acid inhibits renal tubular urate secretion and promotes reabsorption, pharmacodynamically antagonizing probenecid's uricosuric effect.
What to do
Monitor serum uric acid and gout symptoms during antituberculous therapy. Consider an alternative urate-lowering strategy if probenecid control is lost.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Probenecid with Pyrazinamide?
MediHelp rates the interaction as moderate. Probenecid and Pyrazinamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken. Pyrazinamide and its metabolite pyrazinoic acid reduce renal urate excretion, which opposes the uricosuric action of probenecid. Hyperuricemia and gout flares may occur, and the urate-lowering effect of probenecid can be blunted.
How serious is the interaction between Probenecid and Pyrazinamide?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor serum uric acid and gout symptoms during antituberculous therapy. Consider an alternative urate-lowering strategy if probenecid control is lost.
