Summary
Quinidine is a well-known QT-prolonging antiarrhythmic, and voclosporin has been associated with QT prolongation at supratherapeutic exposures. Combined use may add to the risk of torsades de pointes, particularly with electrolyte disturbances or bradycardia. Quinidine is also a P-gp and CYP2D6 inhibitor, and voclosporin is a CYP3A4 and P-gp substrate, so exposure changes are possible.
What this interaction means in plain words
Quinidine and Voclosporin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Quinidine belongs to Anti Parasitic; Voclosporin belongs to Miscellaneous.
Why it happens
Additive prolongation of cardiac repolarization (QTc) through pharmacodynamic synergy.
What to do
Obtain a baseline ECG and monitor QTc after starting the combination, and correct potassium and magnesium before and during therapy. Consider an alternative antiarrhythmic if the QTc becomes significantly prolonged.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
