Summary

Sacubitril/valsartan can cause hypotension and hyperkalemia and may reduce renal function. Together with sotalol, which is renally cleared and QT prolonging, this may increase hypotension, bradycardia and arrhythmia risk through potassium or renal changes.

What this interaction means in plain words

Sacubitril / valsartan and Sotalol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive hypotensive effects, plus angiotensin receptor blockade altering potassium and renal function affecting sotalol clearance.

What to do

Monitor blood pressure, potassium and renal function after starting or changing doses. Adjust the sotalol dose if renal function worsens.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
