Summary

Both the beta-2 agonist and the loop diuretic can lower serum potassium. Used together, especially at higher doses or in patients with heart disease, hypokalemia may become significant and predispose to arrhythmias.

What this interaction means in plain words

Salmeterol and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Salmeterol belongs to Respiratory; Torsemide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive hypokalemia from beta-2 stimulated intracellular potassium shift and diuretic-induced renal potassium loss.

What to do

Monitor serum potassium and magnesium, particularly at treatment start and in patients with cardiac disease. Replace potassium as needed.

What to ask your pharmacist
