Summary
Sarilumab blocks interleukin-6 signalling and is used to manage cytokine release syndrome and inflammatory disease, so it may blunt CAR-T cell activity or expansion. Both agents impair immune defences, which adds to the infection risk in patients who are often already lymphodepleted and neutropenic.
What this interaction means in plain words
Sarilumab and Tisagenlecleucel can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Sarilumab belongs to Musculoskeletal; Tisagenlecleucel belongs to Oncology.
Why it happens
Additive immunosuppression through IL-6 receptor blockade, which may also attenuate CAR-T cell expansion and function.
What to do
Avoid giving sarilumab around the time of CAR-T infusion unless it is needed to treat cytokine release syndrome under specialist direction. Monitor closely for infection and for loss of CAR-T response.
