Summary
Combining a tricyclic antidepressant with an MAO inhibitor such as transdermal selegiline can cause serotonin syndrome and hypertensive reactions. Selegiline labeling advises against use with tricyclic antidepressants, and the risk persists after stopping either agent.
What this interaction means in plain words
Selegiline transdermal and Trimipramine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Inhibition of MAO combined with TCA-mediated inhibition of serotonin and norepinephrine reuptake produces excess monoaminergic activity.
What to do
Do not coadminister. Allow a washout of at least 2 weeks after stopping selegiline before starting trimipramine, and an appropriate washout after stopping trimipramine before starting selegiline.
What to ask your pharmacist
