Summary

Ibutilide markedly prolongs repolarization and carries a substantial torsades de pointes risk. Adding a tricyclic antidepressant with QT-prolonging properties further increases this risk.

What this interaction means in plain words

Ibutilide and Trimipramine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Ibutilide belongs to Cardiovascular; Trimipramine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive prolongation of cardiac repolarization.

What to do

Avoid giving ibutilide to patients on trimipramine; use another cardioversion strategy. If ibutilide is given, continuous ECG monitoring is required.
