Summary
Dofetilide markedly prolongs the QT interval and carries a substantial risk of torsades de pointes. Trimipramine adds further QT prolongation, so the combination is hazardous.
What this interaction means in plain words
Dofetilide and Trimipramine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Dofetilide belongs to Cardiovascular; Trimipramine belongs to Central Nervous System.
Why it happens
Additive QT prolongation through potassium channel blockade.
What to do
Avoid the combination and use a non-QT-prolonging antidepressant. Dofetilide initiation requires in-hospital ECG monitoring.
