Summary

Giving methylphenidate-type stimulants shortly before or during halogenated anesthesia has been associated with abrupt hypertension and arrhythmias in the perioperative period. The sympathomimetic effect of the stimulant may combine with the myocardial sensitization produced by sevoflurane.

What this interaction means in plain words

Serdexmethylphenidate / dexmethylphenidate and Sevoflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive sympathomimetic cardiovascular stimulation from the stimulant on top of volatile anesthetic effects on the myocardium and vascular tone; the exact mechanism is not firmly established.

What to do

Consider withholding the stimulant on the morning of surgery as advised in the product labeling, and resume it postoperatively once the patient is stable. If it has been taken, monitor blood pressure and ECG continuously during anesthesia.
