Summary
Siltuximab blocks interleukin-6, which normally suppresses hepatic CYP enzyme expression during inflammation. As IL-6 is neutralized, CYP activity may return to normal and increase theophylline clearance, lowering its levels. This matters because theophylline has a narrow therapeutic index.
What this interaction means in plain words
Siltuximab and Theophylline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Siltuximab belongs to Miscellaneous; Theophylline belongs to Respiratory.
Why it happens
IL-6 inhibition reverses cytokine-mediated downregulation of CYP1A2 and CYP3A4, increasing theophylline metabolism.
What to do
Monitor theophylline concentrations when siltuximab is started or stopped, and increase the dose if levels drop. The effect may persist for several weeks after siltuximab treatment.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
