Summary

Sofosbuvir-containing regimens, especially those with velpatasvir or ledipasvir, can raise tenofovir exposure, likely through P-gp inhibition by the partner drug. Higher tenofovir levels raise the risk of renal adverse effects, especially when boosted antiretrovirals are also used.

What this interaction means in plain words

Sofosbuvir and Tenofovir disoproxil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of intestinal P-glycoprotein by combination partners (ledipasvir or velpatasvir) increases tenofovir absorption; sofosbuvir alone has little effect.

What to do

Monitor renal function, including creatinine clearance and phosphate, during co-treatment. Consider tenofovir alafenamide if renal risk is high.

What to ask your pharmacist
