Summary

Sorafenib is partly metabolized by CYP3A4, and voriconazole is a strong inhibitor of this enzyme. Exposure to sorafenib may increase, raising the risk of hand-foot reaction, hypertension, diarrhea and hepatotoxicity. The clinical effect is generally modest because CYP3A4 is not the sole route of clearance.

What this interaction means in plain words

Sorafenib and Voriconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Sorafenib belongs to Oncology; Voriconazole belongs to Anti Fungals.

Evidence level: Low

Why it happens

Voriconazole inhibits CYP3A4-mediated oxidative metabolism of sorafenib.

What to do

Monitor for sorafenib toxicity such as skin reactions, blood pressure rise and liver enzyme changes. Reduce the sorafenib dose if adverse effects become intolerable.

What to ask your pharmacist
