Summary
Telmisartan can raise serum potassium, and the blood pressure lowering effect adds to that of sotalol. Hyperkalemia and hypotension or bradycardia may occur, and hyperkalemia can alter cardiac conduction and repolarization. Sotalol is not itself a meaningful cause of hyperkalemia.
What this interaction means in plain words
Sotalol and Telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Sotalol belongs to Cardiovascular; Telmisartan belongs to Cardiovascular.
Why it happens
Additive antihypertensive pharmacodynamic effects, with angiotensin receptor blockade reducing aldosterone-mediated potassium excretion.
What to do
Monitor blood pressure, heart rate and serum potassium, especially at initiation and in renal impairment. Adjust doses if hypotension or hyperkalemia develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
