Summary

Tenecteplase is a fibrin-specific plasminogen activator, and tranexamic acid is an antifibrinolytic that prevents plasmin from acting on fibrin. Together they oppose one another and clot lysis may be diminished. Tranexamic acid can be used purposefully to reverse thrombolytic-related bleeding.

What this interaction means in plain words

Tenecteplase and Tranexamic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Tenecteplase belongs to Cardiovascular; Tranexamic acid belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism: tranexamic acid blocks plasminogen lysine-binding sites, limiting fibrinolysis induced by tenecteplase.

What to do

Avoid concurrent use when thrombolysis is intended. If used to treat major bleeding after tenecteplase, monitor for recurrent thrombosis.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
