Summary
Zotepine has substantial anticholinergic activity, so adding an antimuscarinic such as tiotropium can increase anticholinergic effects. The legacy idea that tiotropium lowers zotepine levels through reduced absorption is not well supported for an inhaled product. Zotepine also lowers seizure threshold and prolongs QT, and olodaterol may add to cardiac effects.
What this interaction means in plain words
Tiotropium-olodaterol and Zotepine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive anticholinergic effects, with possible additive QT prolongation and hypokalemia from olodaterol.
What to do
Monitor for anticholinergic effects such as constipation, urinary retention and confusion. Check ECG and electrolytes if other QT risk factors are present.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
