Summary

Tipranavir, given with ritonavir, can raise tolterodine exposure because tolterodine is cleared partly by CYP3A4, especially in patients who are poor CYP2D6 metabolizers. Higher levels increase anticholinergic effects and the risk of QT prolongation.

What this interaction means in plain words

Tipranavir and Tolterodine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Tipranavir belongs to Anti Virals; Tolterodine belongs to Genitourinary.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4-mediated metabolism of tolterodine by the tipranavir/ritonavir combination, although tipranavir also has mixed induction effects.

What to do

Limit tolterodine to 1 mg twice daily when it is given with strong CYP3A4 inhibitors and monitor for anticholinergic effects and palpitations. Consider an alternative agent in patients with QT risk factors.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
