Summary
Both drugs increase water and fluid loss, so combined use can lead to volume depletion, hypotension and rapid electrolyte shifts. Tolvaptan does not itself raise potassium meaningfully, and the potassium effect mainly reflects torsemide.
What this interaction means in plain words
Tolvaptan and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Tolvaptan belongs to Endocrine; Torsemide belongs to Cardiovascular.
Why it happens
Additive diuretic effects: loop diuretic natriuresis combined with tolvaptan vasopressin V2 antagonist aquaresis.
What to do
Monitor serum sodium, potassium, volume status and blood pressure closely, particularly during initiation. Avoid overly rapid sodium correction and reduce the diuretic dose if dehydration develops.
What to ask your pharmacist
