Summary

Voclosporin, a calcineurin inhibitor, is nephrotoxic, and vancomycin can also injure the kidney. Together they may increase the risk of acute kidney injury and reduced voclosporin tolerability.

What this interaction means in plain words

Vancomycin and Voclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Vancomycin belongs to Anti Bacterial; Voclosporin belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Additive nephrotoxicity from calcineurin inhibitor-induced afferent vasoconstriction and vancomycin tubular injury.

What to do

Monitor serum creatinine and eGFR closely, and check vancomycin levels. Reduce or interrupt voclosporin per labeling if renal function declines.

What to ask your pharmacist
