Mécanisme d'action
Dostarlimab-gxly is a humanized IgG4 monoclonal antibody that binds the PD-1 receptor and blocks its interaction with PD-L1 and PD-L2, releasing PD-1 pathway-mediated inhibition of the immune response, including the anti-tumor immune response.
Posologie adulte habituelle (résumé de la notice)
Primary advanced or recurrent endometrial cancer: 500 mg IV every 3 weeks for 6 cycles with carboplatin and paclitaxel, then 1,000 mg monotherapy every 6 weeks thereafter; until disease progression, unacceptable toxicity, or up to 3 years
Grossesse et allaitement
- Grossesse
- Unsafe Can cause fetal harm: PD-1 blockade can disrupt immune tolerance to the fetus and raise the risk of pregnancy loss; IgG4 crosses the placenta. Check pregnancy status; use effective contraception during treatment and for 4 months after the last dose.
- Allaitement
- Unsafe No data on milk levels. Because of possible serious reactions in the infant, do not breastfeed during treatment and for 4 months after the last dose.
Effets indésirables signalés
- Fatigue/asthenia
- Anemia
- Diarrhea
- Nausea
- Peripheral neuropathy (with carboplatin/paclitaxel)
- Alopecia (with carboplatin/paclitaxel)
Précautions particulières
- Severe and fatal immune-mediated adverse reactions (pneumonitis, colitis, hepatitis, endocrinopathies, nephritis, dermatologic reactions, transplant rejection) can occur in any organ; monitor liver enzymes, creatinine and thyroid function at baseline and periodically.
- Infusion-related reactions: interrupt, slow the infusion, or permanently discontinue based on severity.
- Fatal and other serious complications can occur in patients who receive allogeneic HSCT before or after PD-1/PD-L1 blocking antibody.
