Indications

Mécanisme d'action

Epoetin alfa products stimulate erythropoiesis by the same mechanism as endogenous erythropoietin.

Posologie adulte habituelle (résumé de la notice)

Anemia due to CKD (adults on dialysis or not on dialysis): starting dose 50 to 100 Units/kg 3 times weekly, intravenously or subcutaneously; intravenous route recommended for hemodialysis patients; individualize dosing and use the lowest dose sufficient to reduce RBC transfusions

Résumé de la notice pour la première ligne de posologie uniquement ; les sections complètes sur la posologie figurent dans l'application.

Grossesse et allaitement

Grossesse
Unknown Limited human data; reports in women with kidney disease or anaemia have not reliably shown drug-related harm. In rats, doses near the starting dose caused embryo loss and skeletal and growth effects. Use only if needed; avoid benzyl alcohol (multi-dose) vials.
Allaitement
Safe Erythropoietin is a normal part of milk and the drug is poorly absorbed by the infant. LactMed: no special precautions with preservative-free vials. US label: multi-dose vials (benzyl alcohol) are contraindicated; avoid breastfeeding for 2 weeks after such a dose.

Mises en garde encadrées

  • ESAs increase the risk of death, myocardial infarction, stroke, venous thromboembolism, thrombosis of vascular access and tumor progression or recurrence. CKD: greater risks of death, serious cardiovascular reactions and stroke when targeting hemoglobin above 11 g/dL; no target or dosing strategy avoids these risks; use the lowest dose sufficient to reduce transfusions. Cancer: shortened overall survival and/or increased tumor progression or recurrence in breast, non-small cell lung, head and neck, lymphoid and cervical cancers; use only for anemia from myelosuppressive chemotherapy, not when…

Contre-indications

  • Uncontrolled hypertension
  • Pure red cell aplasia that begins after treatment with RETACRIT or other erythropoietin protein drugs
  • Serious allergic reactions to RETACRIT or other epoetin alfa products

Précautions particulières

  • Increased mortality, MI, stroke and thromboembolism when targeting hemoglobin above 11 g/dL; use caution in coexistent cardiovascular disease and stroke
  • Hypertension: control prior to initiating and during treatment
  • Seizures: increased risk in CKD; monitor for changes in seizure frequency
