Mécanisme d'action
Claudin 18.2-directed cytolytic antibody that depletes CLDN18.2-positive cells via antibody-dependent cellular cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC).
Posologie adulte habituelle (résumé de la notice)
HER2-negative, CLDN18.2-positive gastric/GEJ adenocarcinoma (with fluoropyrimidine- and platinum-containing chemotherapy): first dose 800 mg/m2 IV, then 600 mg/m2 IV every 3 weeks or 400 mg/m2 IV every 2 weeks; continue until disease progression or unacceptable toxicity
Grossesse et allaitement
- Grossesse
- Unknown No human data. No embryo-fetal toxicity in mice at about 1.9 times human exposure, though it crossed the placenta. Labels: give only if the benefit outweighs the potential risk. It is given with fluoropyrimidine and platinum chemotherapy, which can harm the fetus.
- Allaitement
- Unsafe No milk data; as a large antibody little is expected in milk (LactMed). Because of possible serious reactions in the infant, US label: do not breastfeed during treatment and for 8 months after the last dose; EU label: breastfeeding not recommended during treatment.
Effets indésirables signalés
- Nausea
- Vomiting
- Fatigue
- Decreased appetite
- Diarrhea
- Peripheral sensory neuropathy
Précautions particulières
- Hypersensitivity reactions including serious anaphylaxis and serious and fatal infusion-related reactions; monitor during and for at least 2 hours after infusion.
- Severe nausea and vomiting: VYLOY is emetogenic; premedicate with antiemetics before each infusion and manage with antiemetics or fluid replacement.
Informez votre pharmacien ou votre médecin en cas de grossesse ou d'allaitement, de maladie rénale ou hépatique, d'allergies, ainsi que de tout autre médicament ou complément que vous prenez.
