Per cosa si usa
Come agisce
The mechanism in multiple sclerosis is unknown. MMF activates the Nrf2 pathway (response to oxidative stress) and is a nicotinic acid receptor agonist in vitro.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
Relapsing forms of multiple sclerosis: starting dose 95 mg orally twice a day for 7 days, then maintenance 190 mg (two 95 mg capsules) orally twice a day.
Gravidanza e allattamento
- Gravidanza
- Unknown Registry data for dimethyl fumarate (its prodrug) show no rise in birth defects or miscarriage; most exposures were in the 1st trimester. In animals, offspring survival, growth and behaviour were affected at clinically relevant doses. Use only if needed.
- Allattamento
- Safe No direct data, but it is the active form of dimethyl fumarate: low milk levels and data in over 20 infants show no harm, so LactMed finds it acceptable, at least after 1 month of age. US label: weigh benefits. Monitor infant weight gain.
Effetti indesiderati segnalati
- Flushing
- Abdominal pain
- Diarrhea
- Nausea
- Vomiting
- Pruritus
Controindicazioni
- Known hypersensitivity to monomethyl fumarate, dimethyl fumarate, diroximel fumarate, or any excipient (reactions may include anaphylaxis or angioedema).
- Concomitant use with dimethyl fumarate or diroximel fumarate.
Chi deve prestare attenzione
- Anaphylaxis and angioedema can occur after the first dose or at any time; discontinue and do not restart.
- PML: withhold at the first sign or symptom suggestive of PML.
- Herpes zoster and other serious opportunistic infections: consider withholding until the infection resolves.
