Sintesi
Mexiletine is metabolized partly by CYP1A2 and CYP2D6, and fluvoxamine strongly inhibits CYP1A2, so mexiletine exposure can increase. This raises the risk of neurologic and gastrointestinal adverse effects and proarrhythmia.
Cosa significa questa interazione in parole semplici
Fluvoxamine and Mexiletine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluvoxamine belongs to Central Nervous System; Mexiletine belongs to Cardiovascular.
Perché avviene
Fluvoxamine inhibits CYP1A2 (and to a lesser extent other enzymes), reducing mexiletine clearance.
Cosa fare
Reduce the mexiletine dose and monitor for toxicity and ECG changes, or choose an alternative antidepressant that does not inhibit CYP1A2.
Cosa chiedere al farmacista
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
