Bisphosphonate which inhibits bone resorption via actions on osteoclast activity, leading to an indirect increase in bone density.

- Unsafe Not for use in pregnancy (postmenopausal osteoporosis). Bisphosphonates stay in bone for years and may affect fetal bone and calcium; in rats, ibandronate caused embryo loss, difficult labour and kidney effects in offspring. Limited human data show no clear defect pattern.
- 授乳
- Unknown Not indicated in women who could breastfeed; no milk data (present in rat milk). LactMed: very poorly absorbed orally, so infant absorption is unlikely, but an alternative may be preferred, especially with a newborn.

- Severe esophageal risk greater in patients who lie down after taking oral bisphosphonates and/or who fail to swallow it with the recommended full glass (6-8 oz) of water
- Ensure adequate intake of calcium, vitamin D; correct hypocalcemia before use
