Bisphosphonate binds to hydroxyapatite crystals in bone and inhibits osteoclast-mediated bone resorption. Decreases mineral release and collagen or matrix breakdown in bone.

Paget Disease (Initial): 5-10 mg/kg PO qDay not to exceed 6 months OR 11-20 mg/kg PO qDay not to exceed 3 months

Unsafe Not for use in pregnancy. Bisphosphonates stay in bone for years and may affect fetal bone and calcium; animal studies showed skeletal effects, but limited human data show no clear defect pattern.

Unknown No milk data. Very poor oral absorption suggests little infant exposure, but labels do not recommend use while breastfeeding. LactMed: an alternative may be preferred, especially for a newborn or preterm infant.
