Nucleoside Reverse Transcriptase Inhibitor (NRTI); thymidine analog that interferes with HIV viral DNA dependent DNA polymerase and inhibits viral replication

- Unsafe Fatal lactic acidosis has occurred in pregnant women given stavudine with didanosine (a contraindicated combination). Registry data are too few to judge birth-defect risk. Current HIV guidelines no longer recommend stavudine; switch to a preferred regimen.

- Unsafe Passes into milk. US label: do not breastfeed (HIV transmission, viral resistance, infant side effects); LactMed: not a recommended drug during breastfeeding. Follow national HIV guidance; switch to a recommended regimen.
