概要
Cimetidine increases dofetilide plasma concentrations, with greater effect at higher cimetidine doses, and raises the risk of QT prolongation and torsades de pointes. Dofetilide labeling contraindicates concomitant cimetidine. Patients with renal impairment are especially vulnerable.

なぜ起こるのか
Cimetidine inhibits renal tubular cationic secretion (OCT2/MATE) and possibly CYP3A4, reducing dofetilide renal clearance and increasing exposure.
対処法
Do not co-administer. Use an alternative acid suppressant such as a proton pump inhibitor, or famotidine, which is not known to share this effect.
