概要

Prolonged proton pump inhibitor use can cause hypomagnesemia and secondary hypokalemia or hypocalcemia, which increase vulnerability to QT prolongation and torsades de pointes. Domperidone has a dose-related QT-prolonging effect, so electrolyte depletion raises its arrhythmic risk. Risk is highest in older patients, those with cardiac disease and those taking other QT-prolonging drugs or diuretics.

この相互作用をわかりやすく説明すると

Dexlansoprazole and Domperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dexlansoprazole belongs to Gastrointestinal Tract; Domperidone belongs to Gastrointestinal Tract.

エビデンスレベル: Moderate

なぜ起こるのか

Pharmacodynamic: PPI-induced hypomagnesemia (and secondary hypokalemia) potentiates the QTc-prolonging effect of domperidone. No relevant pharmacokinetic interaction is expected.

対処法

Use the lowest effective domperidone dose for the shortest time and correct magnesium, potassium and calcium before starting. Recheck magnesium periodically in patients on long-term PPI therapy, particularly if they also take diuretics or digoxin.
