作用のしくみ

Domperidone is a dopamine antagonist with anti-emetic properties, Domperidone does not readily cross the blood-brain barrier. Domperidone promotes the release of prolactin from the pituitary.

Unknown Limited human data have not shown a rise in birth defects; high, maternally toxic doses harmed rat fetuses. UK label: use in pregnancy only when justified by the expected benefit. Better-studied antiemetics are preferred for pregnancy sickness.

Safe Very low milk levels (under 0.1% of the mother's dose) and no infant harm in published cases; it is used to boost milk supply. It carries a heart rhythm risk for the mother. UK label: decide between breastfeeding and the drug, as infant heart effects cannot be excluded.

禁忌

  • Hypersensitivity to domperidone or any formulation ingredient.
  • Known prolongation of cardiac conduction intervals, particularly QT; significant electrolyte disturbances; underlying cardiac disease such as congestive heart failure.
  • Moderate or severe liver impairment.

注意が必要な方

  • May be associated with serious ventricular arrhythmias or sudden cardiac death; risk may be higher in patients over 60 or at daily doses above 30 mg. Use the lowest dose for the shortest duration.
  • Patients at higher risk of QT prolongation or arrhythmia should be considered for cardiac assessment before and during treatment.
  • Moderate CYP3A4 inhibitors such as diltiazem, verapamil and some macrolides are not recommended with domperidone.
