概要
Posaconazole substantially increases finerenone exposure, raising the risk of hyperkalemia and hypotension and worsening renal function.

Finerenone and Posaconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Posaconazole strongly inhibits CYP3A4, the main route of finerenone metabolism.
対処法
Do not use together; labeling contraindicates strong CYP3A4 inhibitors. If antifungal therapy is needed, hold finerenone for the course and check potassium before restarting.
