摘要
Amiodarone and atenolol both depress sinus node and AV conduction, so combining them can cause marked bradycardia, AV block or sinus arrest. Risk is greatest in older patients or those with conduction disease, and amiodarone's long half-life prolongs the risk.
用白話說明這項交互作用的意義
Amiodarone and Atenolol-chlorthalidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amiodarone belongs to Cardiovascular; Atenolol-chlorthalidone belongs to Cardiovascular.
為什麼會發生
Additive negative chronotropic and dromotropic effects through beta-blockade and amiodarone's electrophysiologic actions.
處置方式
Monitor heart rate and ECG closely when starting or adjusting either drug, and reduce the atenolol dose or stop therapy if symptomatic bradycardia or conduction block occurs.
