摘要
Asenapine blocks dopamine receptors and may antagonize the antiparkinsonian effect of levodopa, while both drugs can cause orthostatic hypotension. The combination may worsen parkinsonian symptoms and increase dizziness or falls.
用白話說明這項交互作用的意義
Asenapine and Carbidopa-levodopa can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Asenapine belongs to Central Nervous System; Carbidopa-levodopa belongs to Central Nervous System.
為什麼會發生
Pharmacodynamic antagonism at dopamine D2 receptors, plus additive orthostatic hypotension through alpha-1 blockade and levodopa effects.
處置方式
Avoid in Parkinson disease if possible, preferring low-D2-affinity agents such as quetiapine or clozapine when an antipsychotic is needed; monitor motor function and orthostatic blood pressure.
