摘要
Valproate increases oxybate exposure, which can intensify respiratory depression, sedation and coma. Oxybates already carry serious CNS and respiratory depressant risks.

Valproate inhibits GHB dehydrogenase and possibly monocarboxylate transporters, reducing oxybate clearance, with additive CNS depression.
處置方式
Reduce the nightly oxybate dose by at least 20% when valproate is started, or use a lower oxybate starting dose in patients already on valproate. Titrate by response and monitor for respiratory and CNS depression.
