Summary

Hyperkalemia caused by amiloride may increase the cardiac toxicity of antiarrhythmics such as disopyramide, with conduction slowing. Disopyramide itself may also reduce blood pressure and contractility.

What this interaction means in plain words

Amiloride and Disopyramide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amiloride belongs to Cardiovascular; Disopyramide belongs to Cardiovascular.

Evidence level: Low

Why it happens

Potassium retention from amiloride may exaggerate the conduction-slowing effects of class I antiarrhythmics, in addition to additive cardiovascular depression.

What to do

Monitor serum potassium and ECG, and correct hyperkalemia promptly. Review blood pressure and heart rhythm after starting.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
