Summary

Disopyramide is a class Ia antiarrhythmic that prolongs the QT interval and has caused torsades de pointes. Amisulpride adds further QT prolongation, so the combination markedly increases proarrhythmic risk.

What this interaction means in plain words

Amisulpride and Disopyramide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Amisulpride belongs to Central Nervous System; Disopyramide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive QT prolongation through potassium channel blockade.

What to do

Avoid the combination and select an antipsychotic or antiarrhythmic with lower QT risk. If unavoidable, monitor the ECG closely and correct potassium and magnesium.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
