What this interaction means in plain words

Amiodarone and Tacrolimus can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amiodarone belongs to Cardiovascular; Tacrolimus belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Additive QTc prolongation, with possible amiodarone inhibition of CYP3A4 and P-gp increasing tacrolimus exposure.

What to do

Monitor tacrolimus trough levels and adjust the dose, and monitor ECG and electrolytes. Check renal function regularly.
