Summary

Pentobarbital is a barbiturate with strong CNS depressant effects. With amisulpride, sedation and psychomotor impairment can be markedly increased, and hypotension or respiratory depression is possible at higher barbiturate doses. Barbiturates can also lower the seizure threshold on withdrawal, and antipsychotics may lower it as well.

What this interaction means in plain words

Amisulpride and Pentobarbital can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive CNS depression via GABA-A potentiation by pentobarbital and the sedative effects of amisulpride.

What to do

Use the lowest effective doses and monitor level of consciousness, respiratory status and blood pressure closely. Avoid use outpatient unless supervised.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
