Summary
Dofetilide is a class III antiarrhythmic with a substantial risk of torsades de pointes through QT prolongation. Amisulpride adds to this effect and may lead to life-threatening arrhythmia.
What this interaction means in plain words
Amisulpride and Dofetilide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Amisulpride belongs to Central Nervous System; Dofetilide belongs to Cardiovascular.
Why it happens
Additive QT prolongation by blockade of the rapid delayed rectifier potassium current.
What to do
Avoid concurrent use and choose an antipsychotic with minimal QT effect. If treatment cannot be changed, ensure continuous ECG monitoring and normal electrolytes.
