Summary
Combining an ACE inhibitor with a thiazide diuretic produces additive blood pressure lowering, which is often intended but can cause symptomatic hypotension, especially in volume-depleted patients or at initiation. Renal function may decline and electrolyte disturbances can occur.
What this interaction means in plain words
Amlodipine-benazepril and Hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-benazepril belongs to Cardiovascular; Hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Diuretic-induced volume and sodium depletion activates the renin-angiotensin system, so ACE inhibition produces an exaggerated fall in blood pressure and reduced glomerular filtration pressure.
What to do
Monitor blood pressure, serum creatinine and electrolytes after starting or adjusting either drug. Correct volume depletion and consider reducing the diuretic dose before initiating the ACE inhibitor.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
