Summary
Metolazone with benazepril produces additive blood pressure lowering and can cause marked hypotension, especially in volume-depleted patients. Metolazone is potent and may also cause significant electrolyte loss and reduced renal function when combined with an ACE inhibitor.
What this interaction means in plain words
Amlodipine-benazepril and Metolazone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-benazepril belongs to Cardiovascular; Metolazone belongs to Cardiovascular.
Why it happens
Diuretic-induced sodium and volume depletion increases dependence on angiotensin II, so ACE inhibition causes exaggerated hypotension and reduced glomerular filtration.
What to do
Monitor blood pressure, serum creatinine, sodium and potassium after starting or adjusting either drug. Consider a lower starting dose of the ACE inhibitor in volume-depleted patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
