Summary

Esmolol and the three antihypertensives in this combination have additive blood pressure lowering effects, which can cause hypotension and bradycardia, especially during intravenous esmolol infusion. Beta-blockers can also slightly raise potassium, and amlodipine may add to negative chronotropic and inotropic effects. Valsartan and hydrochlorothiazide are not for use in pregnancy, which is a separate concern.
