Summary
Ethacrynic acid is a potent loop diuretic and hydrochlorothiazide is a thiazide, so together they produce additive potassium and magnesium loss and sodium depletion. This may cause hypokalemia, arrhythmias, volume depletion and hypotension. The valsartan component partly offsets potassium loss but does not remove the risk.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Ethacrynic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Ethacrynic acid belongs to Cardiovascular.
Why it happens
Additive renal potassium and sodium wasting from sequential nephron blockade by loop and thiazide diuretics.
What to do
Monitor serum potassium, magnesium, sodium and renal function, especially at initiation and dose changes. Replace electrolytes as needed and watch for volume depletion and hypotension.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
